HOW DO WE USE COA QUESTIONNAIRES?

Stakeholder questionnaires are used by COA Endorsers to learn how key stakeholders perceive your program and experience its services.

You will be asked to distribute these questionnaires to stakeholders 4-5 months prior to your scheduled site visit date.Results of these questionnaires are not made available to programs prior to the site visit. Programs can request the results after the completion of the site visit by contacting their after school Accreditation Coordinator.

While you are welcome to also use these questionnaires for stakeholder involvement and input into your program improvement process, you are not required to use this specific tool. As such, you are welcome to modify this tool for your individual program needs. In addition, the use of these questionnaires as part of your program improvement process will not negate the requirement for you to implement them in accordance with COA processes during the accreditation self study as mentioned above.
