HOW DO WE USE THE COA QUESTIONNAIRES?

Stakeholder observations and questionnaires are used by COA to learn how key stakeholders perceive your program and experience its services.

You will be asked to distribute these questionnaires to stakeholders so they can complete them either online or by telephone. Complete instructions for distributing the questionnaires and postcards to introduce the questionnaires can be found in the Related Files. In addition, programs should have stakeholders conduct program observations. The program observation tool can be found in the "Tools Index".
